Medical Intuitive/Intuitive Readings Disclaimer and Waiver
Hello, and Welcome to Leslie Anne Malach, The Evidential Medium!
When you book a reading with me, you will be asked to download and sign a consent form and waiver that explains the nature of my business and its limits. It's important that you read through this section in its entirety, and once you agree to the policies, and sign the consent form and waiver, your session will be scheduled. I’m including it here and stating that by way of booking or participating in your appointment, you agree to the following:
That you are at least 18 years old and have voluntarily decided to book or participate in a reading for yourself. I do not read for people under 18 years of age without parental consent.
That you understand that an intuitive/psychic reading is in no way a substitute for professional medical, mental health, spiritual counseling, legal or financial advice and/or treatment. You are responsible for seeking appropriate care for mind and body. I am a Licensed Massage Therapist, Certified Reiki Master/Teacher, Health and Wellness Coach, Forgiveness Coach, Trained Evidential Medium, Trained Animal Communicator and have advanced training as a Medical Intuitive. I strongly urge you to be your own healer, your own best advocate and I am able to give recommendations and guidance to help you on your healing path; body, mind, and spirit, with the many resources I’ve acquired, but have no affiliation with. I, Leslie Anne Malach, do not diagnose, prescribe, or give medical advice. It is your responsibility to check with your medical provider with questions.
That you understand that I, or any intuitive/psychic, cannot guarantee 100 percent accuracy of any statement and that I do not include or guarantee predictions of the future in my practice. I always urge people who see me to remain authorities in their own lives. No psychic, medium, intuitive, or other counselor can tell you what to do or tell you who you are. Intuitives, psychics, and mediums and other such practitioners can be wrong, can interpret material incorrectly, or put language to the information they receive that miscommunicates, or only partly communicates, what they are seeing, sensing, hearing. Remember that intuitives encounter complex symbols, feelings, tastes, sounds, images, memories, and so on, which they then must rapidly translate into human language. Error is possible. How you interpret the information is solely your responsibility.
That you understand that you may receive information that you may consider unpleasant, painful, or emotionally difficult. Traumatic events often come up in a reading as a cause of physical illness, or chronic situations. If difficult information comes up in a reading, you’ll be asked if you want to receive that information. It is your responsibility to answer this honestly.
That I, Leslie Anne Malach, will not read another person without their consent, regardless of their age or your relationship to them. I do not do 3rd party readings.
That any healing during a session is the permanent release of a burden and a cure is NEVER guaranteed. You assume sole responsibility for your health, health care, and well-being.
Any reading or treatment/s will not replace conventional medical diagnoses or treatment. You agree to continue taking any medication prescribed by your licensed medical provider and will continue to follow their instructions.
Confidentiality: All sessions are private and will never be discussed, unless I receive written consent, from you, to do so. Neither you or I will record a session without permission, or publish any part of the session. Please note that Zoom, Skype, FaceTime, Google Hangouts, Dropbox, email, texts, another digital platforms may not be completely confidential in themselves and use of these platforms does entail certain risks. I will NEVER share your information.
I understand that Leslie Anne Malach, will end the session if I feel you are playing “Stump the Psychic”, which is intentionally blocking the reader, preventing the intuitive from reading you. This is not conducive to either of us, and a refund will not be given.
I understand that I may experience detoxing symptoms following the session which may include but are not limited to; increased urination, diarrhea, exhaustion, thirst, headache, runny nose, increased energy.
POLICY AGREEMENT
I strive to work ethically, to treat people with respect and dignity. I also understand that my style of reading is to be very free, open to spirit and our Divine and Sacred Guides, to unconscious influences, and that I may use language that you may misunderstand or dislike. I strongly urge you to seek an intuitive with whom you feel comfortable, whose style resonates with you. If I am not that person, it is my wish and hope for you that you find exactly the right fit with a person who can help you blossom!
Consent for care: It is my choice to participate in this healing session, and I give consent to receive treatment. Clients under the age of 18 will require a parent or legal guardian to sign the consent form. By signing below, you are acknowledging that you have read, understand, and agree with the policies of Leslie Anne Malach.
All sessions require pre-payment to secure your appointment time. A 24 hour notice is required to reschedule your session. Refunds are not given for cancellations. With a 24 hour notification of cancellation, the appointment will be rescheduled. A no show WILL NOT be refunded or be rescheduled. This appointment time has been reserved for you and only you. Please be on time, the session will remain open only 5 minutes past the scheduled time and will be considered a no show at that point. A full payment will be required to reschedule your reading.
